ABOUT THE SPEAKER

Dr. Michael has been a practicing physician for more than 25 years. In addition to developing a thriving optometric medical practice, he has taken on a variety of leadership roles in the optometric industry as a member of the American Optometric Association (AOA) and has served in numerous capacities (including president) for the Optometric Physicians of Washington (OPW).

Advanced Strategies was born out of Dr. Michael's career-long passions in pursuing the logical integration of vision and medical services, improvement of patient care, and defining business strategies that will significantly increase and optimize cash flow from optometric services.

As optometrists we have all struggled during the past several years to increase or to even maintain our current profitability. Several forces are working against our efforts at profitability. Some of these are:

1) A significant amount of our vision practice income is increasingly dependent on discounted insurance plans such as Davis, EyeMed, Cole, Superior Vision and Spectra.

2) Overtime these discounted vision plans have come to dominate as the primary source and payer for our vision services.

3) These and other discounted plans have continued to pay us minimally for our services and frequently at levels below what Medicaid pays.

4) Payments from these and other discounted vision plans are often determined by methods that seem confusing, contradictory and are often shrouded in mystery.

5) Reimbursements from these discounted vision plans often seem designed to pay us as little as they can get away with.

6) Many of these discounted vision plans have now paid the same amount for our services year after year, often with little or no real hope for increases in reimbursement.

7) Overtime these discounted vision plans have placed additional limitations on how we can charge and collect for our services further limiting opportunities for profit.

8) Practice expenses each year have continued to rise while reimbursements for our services have remained static or decreased.

9) Static and declining income combined with escalating practice expenses is resulting in cash flow problems which overtime is a recipe for our financial disaster.

10) Increased competition in the area of optical goods has led to further erosion of our profitability as more of our patients choose to fill the prescriptions that we write elsewhere.

11) In an effort to remain independent of these discounted vision plans many have opted to pursue a course of building their practice based on the private pay patient.

12) In pursuing the private pay patient many feel they must price their vision care services low enough in order to be competitive with the commercial outlets.

13) Pricing our services low enough to be competitive with the commercial outlets further decreases net profit.

14) In response to declining cash flows and in an effort to pay for this shortfall in profit many attempt to see more patients in less time while at the same time reducing necessary expenses often including their own compensation. This is called working harder for less and is not a smart business strategy for the long term.

15) Pursuing such a business strategy ultimately plays into the marketing plans of our competition which is a no win plan for us.

16) All of these forces will result in less profits, more stress, less joy and will ultimately end in more burnout for us and our staff. It is the strategy of working harder not smarter and is never very much fun for very long.
